Describe the symptoms/problems keeping the following questions in mind: Why is it being brought to the shop? Has this problem happened before? If yes, when? Where? Where was the unit when the problem occured? How was it used or setup? What was it asked to do? Surrounding conditions - were they different? Has anyone tried to repair it? What is the name of the equipment, model, etc.? Is there a repair or owner's manual available?

Description of work requested or apparatus/instrument required. Please give a basic description of the intended use or purpose of instrument. Include specifications and use a diagram if necessary. Also include any proposed solution if available. A meeting with a member of our staff may be needed to discuss details before work can begin.
